1. Importer of record number

a. Internal Revenue Service (IRS) number, Employer Identification Number (EIN), Social Security Number (SSN) or CBP assigned
number of the entity  liable for the payment of all duties and responsible for meeting all statutory and regulatory requirements incurred as a result of importation.

b. For goods intended to be delivered to a Foreign Trade Zone or FTZ, an IRS number, EIN, SSN or CBP assigned number of the party filling the FTZ documentation with CBP must be provided.
The importer of record number for ISF purposes is the same as "importer number" on CBP Form 3461.

2. Consignee number

a. Internal Revenue Service (IRS) number, Employer Identification Number (EIN), Social Security Number (SSN) or CBP assigned
number of the individual(s) or firm(s) in the United States on whose account the merchandise is shipped. This number is the same as "importer number" on CBP Form 3461.

3. Seller (Owner) name/address

a. Name and address of the last known entity by whom the goods are sold or agreed to be sold. If imported otherwise than in pursuance of a purchase, the name and address of the owner
must be provided.

b. The name and address is the same information required for the invoice of the imported merchandise. (See 19 CFR 141.86 (a)(2).

c. Instead of the name and address, CBP will accept a Dunn and Bradstreet number (DUNS)

4. Buyer (Owner) name/address

a. Name and address of the last known entity to whom the goods are sold or agreed to be sold. If imported otherwise than in pursuance of a purchase, the name and address of the owner
must be provided. If imported otherwise than in pursuance of a purchase, the name and address of the owner must be provided.

b. The name and address is the same information required for the invoice of the imported merchandise. (See 19 CFR 141.86 (a)(2).

c. Instead of the name and address, CBP will accept a Dunn and Bradstreet number (DUNS)

5. Ship to Party

a. Name and address of the first deliver-to party scheduled to receive the freight after release from customs custody.
   (1) Provide the actual name and address the goods are being delivered to and not the corporate office.
   (2) If the deliver-to address is not known, then the name of the facility where the goods will be unladen may be provided..
   (3) A FIRMS code for the terminal, pier, warehouse or CFS station may be provided if the ship-to and address is not known at the time of the ISF filing.
   (4) An in-land distribution center may be provided if the specific ship-to name and address is unknown at the time of the ISF filing..

b. Instead of the name and address, CBP will accept a Dunn and Bradstreet number (DUNS)

6. Manufacture (Supplier) name/address

a. Name and address of the entity that last manufactures, assembles, produces or grows the commodity.
b. Or, name and address of the supplier of the finished goods in the country from which the goods are leaving.
c. Another acceptable alternative is the name and address of the manufacture or supplier that is currently required by the import laws, rules and regulations of  the United States.
    This is the same information used to create a Manufactures ID or MID number for entry purposes.
d. Instead of the name and address, CBP will accept a Dunn and Bradstreet number (DUNS)

7. Country of origin

a. Country of manufacture, production or growth of the article based upon import laws, rules and regulations of the United States.
This is the same as the country of origin on CBP Form 3461.

8. Commodity HTS tariff number (Min. 6 digits, max. 10)

a. Tariff number under which the goods are classified in the Harmonized Tariff Schedule of the United States.
The tariff number must be provided at least to the sixth digit but CBP prefers the entire 10 digit number.

9. Container stuffing location

a. Name(s) and address(es) of the party(s) where the goods were physically stuffed into the container. For CFS (less than a container load) shipments,
the name and address of the party must be provided who made the goods "ready to ship" or the party who arranged for the goods to be made "ready to ship"
b. Instead of the name and address, CBP will accept a Dunn and Bradstreet number (DUNS)

10. Consolidator name/address

a. Name and address of the party who stuffed the container or arranged for the stuffing of the container. For CFS shipments,
the name and address of the party must be provided who made the goods "ready to ship" or the party who arranged for the goods to be made "ready to ship"
b. Instead of the name and address, CBP will accept a Dunn and Bradstreet number (DUNS)
